HealthChoices is the Medical Assistance (Medicaid) program for Pennsylvania. In HealthChoices, you choose a primary care provider (PCP) and health plan for each family member.
Your PCP is your doctor or other provider who gives you most of your regular health care.
A health plan is a group of doctors, hospitals, clinics and other health care providers. They work together to meet your health care needs.
Here are the steps to help you choose:
Step 1 – Choose your PCP
Choose a doctor or other provider to be your primary care provider (PCP).

- To search for a PCP, go to Find a provider.
Step 2 – Choose your health plan
You must choose a health plan that works with your PCP.

- To compare health plans in your county, go to Compare plans.
Step 3 – Enroll in one of these ways
- Online
- By phone
- By mail
