Enrolling is important! You can enroll online, by phone or by mail.

 

 

1. Learn about health plans and doctors. (You may skip this step if you know the health plan and doctor you want.)

2. Choose a doctor that works with the health plan you choose.

3. Enroll online, on paper or by phone. To enroll online you will need your Recipient ID number.

  • Enroll now online. (You can log in and see your choices before you enroll. Your enrollment is not final until you press "Save and enroll.")

  • Enroll over the phone; call 1-800-440-3989, or

  • Enroll by mail. You will get a packet in the mail. You can fill out the enrollment form in the packet and mail it in the return envelope.

After you enroll

Your health plan will send you an ID card and a member handbook. The handbook will tell you what you need to know to get health care services. Please read it carefully and call your health plan if you have questions.
